Biographies

Zoe Carter

Zoe is a Consultant Nurse in Frailty within Primary Care and Community Services at Aneurin Bevan University Health Board. With experience spanning acute, community, and emergency care, she provides strategic leadership to enhance frailty pathways across community services and the urgent and emergency care interface. Zoe has led the development of innovative nurse‑led models within a community ward and front‑door frailty responses in A&E and AMU. She is passionate about advancing the nurse practitioner portfolio within frailty, strengthening service integration, and delivering person‑centred care closer to home.

Amanda Powell

I am currently the Lead Pharmacist for Community Resource Teams (CRTs) and Frailty in Aneurin Bevan UHB. Over the last 6 years we have developed a service, helped by the ‘A Healthier Wales’ agenda, funded by the Welsh Government’s Regional Integration Fund.

I have previously worked in hospital and primary care pharmacy and have taught on the prescribing course in Cardiff University.

I believe that pharmacy should work completely within the MDT contributing to decision making because we are there at the point of decision making and care planning. I lead a team of Pharmacists, all of whom are active prescribers within the MDT. Our Pharmacy Technicians, are working at the top of their licence, interacting within consultant led virtual and actual ward rounds within Frailty and Rehabilitation.
